Volunteer Informed Consent Agreement

Release of Liability, Assumption if Risk & Agreement to Pay Claims

 

In working with the Prison Education Project, which is under the auspices of Transcendence Children & Family Services, I understand and agree to the following statements.

 

1. I am at least 18 years of age.

2. I am responsible for my own transportation to and from the volunteer site. The commute is my sole responsibility.

3. I am responsible for evaluating the safety of the volunteer site. Any concerns should be brought to the attention of the community-based organization’s site manager or supervisor.

4. I voluntarily assume any and all risks and agree to pay any and all claims arising from or incidental to my participation in volunteer service activities/events including those that may result in:

   1. Personal injuries including medical or hospital bills, permanent or partial disability or death.

   2. Damage to personal property caused by or arising from participation in the volunteer service.

5. I will submit to the community-based organization’s required background check and respect their rules of privacy.

6. I grant the Prison Education Project the right and permission to use photographs, film, tape or sound recordings taken of them during the course of their volunteer service for use in non-print and print media presentations.

7. I may be removed from participation at the community-based organization’s site with or without cause.

8. The Prison Education Project has no control over how the information they have provided will be used by third parties.

9. I will abide by “Participant Conduct Guideline and Limitations”.

10. I will participate in all relevant trainings required by the Prison Education Project.

11. I will engage in model professional and appropriate behavior when working with clients and when on community-based organization site.

 

As a condition of using the volunteer service, I release the Prison Education Project and Transcendence Children and Family Services and their employees, officers and agents from Liability for personal injury, wrongful death and property damage arising from participation in any events or activities associated with the Prison Education Project.  The attached “Volunteer Participation Guidelines and Limitation”, consisting of one page, is incorporated by reference and made a part of this agreement.

 

This document reflects my understanding of the relationships. My electronic signature and submission on the registration page confirms my understanding and consent for release of liability.
